PEO Renewal: How Contract and Health Insurance Renewals Work (2026)

What is a PEO renewal? A PEO renewal is really two renewals. Your contract renewal covers the service agreement and admin fee, and most PEOs run month to month with 30 days' notice. Your health insurance renewal resets benefit rates once a year on the PEO's plan year, based on how its whole book and your own company performed.

I spent more than ten years inside three PEOs, and most clients I talk to only think about one of these. They negotiate the admin fee hard, sign, and then get surprised by a health insurance renewal they didn't see coming. This guide walks through both kinds of PEO renewal, how each one is priced, when the big PEOs renew, and how to get ready. For the full picture of how PEOs charge, start with our PEO pricing guide.

What are the two types of PEO renewal?

Contract renewal Health insurance renewal
What renews Your client service agreement and admin fee Your medical, dental and vision rates
How often Usually month to month; annual if you commit to a term Once a year, on the PEO's plan year
What drives the price Your negotiation, headcount and term The PEO's whole book of business, then your own company's claims
How to leave Usually 30 days' written notice Leave the PEO, ideally timed to the plan year
PEO renewal explained: the contract renewal covers the service agreement and admin fee, usually month to month with 30 days' notice, while the health insurance renewal resets rates once a year based on the PEO's book of business and your company's claims
Two renewals, two different sets of numbers to watch.

How does a PEO contract renewal work?

Most PEOs don't lock you into a long-term contract. In my experience, the standard client service agreement runs month to month. Give the PEO 30 days' written notice and you can leave. There's no big annual renewal event for the contract itself, because it renews quietly every month you stay.

That's changing at the edges. It's becoming more common for PEOs to offer a lower admin fee if you commit to a full year. That trade makes sense on both sides. The PEO gets a client it can count on, and you get a better rate for a commitment you were probably going to keep anyway.

Before you take an annual deal, check three things in the agreement:

  • What it locks in. An annual commitment usually locks your admin fee. It doesn't lock your health insurance rates, which still renew on the PEO's plan year.
  • What leaving early costs. Look for an early termination fee or a requirement to pay out the rest of the term.
  • How it renews. Find out whether the term renews automatically and how much notice you need to give to stop it.

Whether you're month to month or on a term, your notice period is the clause that matters most at renewal. Miss it and you can be stuck for another cycle. For every clause to check before you sign, from notice periods to early termination fees, see our PEO agreement guide. Our guide to average PEO fees for small businesses shows what a fair admin fee looks like before you negotiate.

How does a PEO health insurance renewal work?

This is the PEO renewal that moves the most money. It happens once a year, and it happens in two steps.

Step 1: The whole book renews

First, the PEO's entire book of business gets renewed. The PEO and its carriers look at how the health plan performed over the past year, mainly its loss ratio: how much went out in claims compared with how much came in as premium. A book running hot gets a bigger base increase. A book running well gets a smaller one.

The big PEOs' own numbers show how much this matters. TriNet reported a 91% insurance cost ratio for 2025, meaning 91 cents of every insurance dollar it billed went to insurance costs. Insperity said its 2025 results were hit by elevated benefits costs and partly offset by higher pricing. When the book costs more, every client feels it at renewal.

Step 2: Your company gets its own renewal

Then each company gets an individual renewal based on its own performance. In my experience, the PEO starts from the book's base increase and adjusts it for your group: your claims over the past year, any large claims, and changes in your census like a team that's getting older. A healthy group can come in below the book's increase. A group with a rough year can come in well above it.

How a PEO health insurance renewal works in two steps: the PEO's whole book of business renews based on its claims loss ratio, then each company gets its own renewal based on its own claims and census
The book sets the baseline. Your company's own claims move you up or down from there.

The market backdrop matters too. Employers expect health benefit costs to rise 8.2% on average in 2027, the biggest jump since 2003, according to Marsh's 2026 survey of more than 1,800 employers (the survey formerly run under the Mercer name). A PEO renewal near that number is the market moving. One well above it deserves questions. Our guide to PEO underwriting explains how PEOs price your group in the first place.

When are PEO renewal dates?

Your health insurance renewal follows the PEO's plan year, not the date you signed. Here are the plan years for three of the biggest PEOs:

PEO Health plan year starts When the work starts
ADP TotalSource June 1 (runs June 1 to May 31) Open enrollment in the spring, before June 1
Justworks November 1 Employers chose plans August 11 to September 1, 2026; employees enrolled in September
TriNet Four plan years: January 1, April 1, July 1 or October 1 A "Benefits Strategy Solutions" period about three and a half months before your plan year

Two things follow from this. First, if you join mid-year, your first plan year may be short, and your first renewal can come only a few months after you sign. Second, the best time to leave a PEO is usually at its plan year, so your team doesn't switch plans mid-year and restart deductibles. We cover that in what happens when you leave a PEO.

How to prepare for your PEO renewal

  1. Know your dates. Write down your plan year start, when your renewal arrives, and your contract's notice period.
  2. Ask for the renewal early. You want time to compare before the plan selection window closes. At TriNet that window opens about three and a half months out. At Justworks, employers picked plans more than two months before the plan year.
  3. Ask how the increase breaks down. How much came from the PEO's book, and how much from your own company? If your group drove it, ask why.
  4. Get competing quotes. A real quote from another PEO, or from the open market, is the strongest negotiating tool you have.
  5. Negotiate the admin fee at the same time. If your health rates are going up, it's a fair moment to ask for a better fee or an annual-commitment discount. Our guide to PEO discounts covers every lever you can ask for, including a renewal cap.
  6. Revisit your plan lineup and contributions. Sometimes a different plan mix softens an increase without cutting coverage your team uses.

That's the work a PEO broker does before every renewal: pulling apart the increase, getting competing quotes and pushing back where the numbers don't hold up.

Frequently asked questions about PEO renewal

How much do PEO renewals go up?

It depends on the PEO's book and your own claims. For context, employers expect health benefit costs to rise 8.2% on average in 2027, per Marsh. Your PEO renewal can land above or below that.

Can I leave my PEO before the renewal?

Usually, yes. Most PEO agreements are month to month and need about 30 days' written notice. If you signed an annual commitment for a lower fee, check for early termination costs first.

Can I negotiate my PEO renewal?

Yes. Ask how the increase breaks down, bring a competing quote, and negotiate your admin fee at the same time. PEOs have more room on renewals than most clients assume.

When will I get my PEO renewal?

Before your PEO's plan year starts. TriNet opens its plan selection period about three and a half months before each plan year. Justworks clients chose plans in August for a November 1 start. Ask your PEO for the exact date.
